Cervical Spondylotic Myelopathy: Anterior Approach: Multilevel Anterior Cervical Discectomy and Fusion versus Corpectomy

Cervical spondylotic myelopathy is one of the most common spinal disorders in older patients. Because of its generally progressive course, surgical treatment is usually recommended. Although the pathology may be focal, it is more commonly diffuse and surgery must therefore address the multilevel nature of the condition. The surgical options therefore include either multilevel anterior discectomy and fusion or corpectomy. This article will examine the rationale for each of these procedures as well as technical considerations, complications and the authors’ preferred technique.
